When you are ready to enter your team into the Christopher Columbus Awards competition, please complete the entry form, print it and return it with the entry.

Entry forms must be submitted by mail; they cannot be submitted electronically or via fax.

• Please fill out each field before printing the next page.
• Before mailing the completed form, remember to have the coach and team members each sign the form. Entries will not be accepted without signatures.
• Mail Completed Entries to:

• Note: Mail one original and three photocopies of the completed entry (four total).
• Faxed entry material will not be accepted.
• Entries must be postmarked by March 17, 2008, or the entry will be ineligible for the competition.
• Materials that do not meet the rules and regulations will be disqualified.
• Permission is granted to duplicate this Official Entry Form.
